To everyone that has been acquiring reaching out to the Mario Reed Foundation about assistance and resources such as the Choices Program, and other state organizations resources to help people with paralysis and disabilities. Here are a few below to start with.

Anyone who wants to make application can call (901) 222-4111 and they will assist
in getting started. As you know TennCare determines who is eligible for Choices. The Aging Commission assist persons in making application gather information required by TennCare and then forward on to them.
